Applicants can download the official registration form from either the OPM or PAGCOR
websites (http://www.opm.org.ph or http://www.pagcor.ph). Registration forms may also
be secured free of charge from any PAGCOR casino branch nationwide. |

Applicants must submit complete application package including accomplished registration
form, documentary evidence or proof of citizenship and age such as scanned or photocopied passport or NSO-authenticated birth certificate (in the absence of passport);
and any scanned or photocopied government-issued valid ID such as driver’s license, SSS or GSIS ID, postal ID, or school or company ID. |

All applications should be received on or before the deadline at 11:59 p.m. of March 15,
2011 to qualify for auditions. Applications with incomplete data, form or documentation
requirements will automatically be rejected. |

All applicants who will qualify will be notified by email, snail mail and/or SMS advice to
report for auditions and actual regional competitions in the following venues and schedule: |
